So I had a day to do some tweaking around with the config files to try and get the most realistic settings I could, as i found Nico's settings to be a bit overpowered. Only to find I also had the weird standing around while bleeding at 4 health bug mentioned in previous comments.
Turns out its not a bug and just a weird behavior between PDO's "OnlyBleedOnTorsoShot" ini setting and the way the bloodlust mod handles bleeding.
Sometimes a torso shot that causes an arterial bleeding through bloodlust (Bloodlust changes the vanilla bleedouts) doesn't register as a torso shot hit through PDO.
The npc will then go through the running while bleeding animation only to stop at 4 heath and refuse to die due to the "OnlyBleedOnTorsoShot" ini setting (It doesnt think its been hit in the torso)
I've fixed this issue by editing the collapse time in the \lml\WhyEm's BloodLust\bloodHaemorrhage\peddamageinfo.meta file
No longer should you see peds standing around bleeding.
I also feel I've tweaked my PDO config to an almost perfect state (in my opinion) for the most realism getting the most out of the features from both mods
Some features of my tweak
-Legshots no longer do 1 damage, peds wont infinitely survive getting shot in the legs. 2-5 shots to legs will put them in a dying state (like getting your legs shot up would)
-Spine Shots cause paralysis is enabled (rare). I find many versions have this disabled
-Peds bleed out fast only if they have been hit in the torso, otherwise legs, arms and neck shots only will cause a longer 2 minute bleed out to simulate light and heavy bleeding
-Peds will go through the full 5-30 second running bleed from bloodlust and fall at the end
-All body part damages have been tweaked to something more realistic
-Very rarely peds will survive a headshot if not hit in the brain with weaker weapons (Unless using headbleed then its quite often)
-Lasso and hogtie disarms are enabled. These are not enabled in many other versions even though it says they are
-Melee combat can put peds into dying states
-Special Weapon damage has been only slightly increased (shotguns,snipers,dynamite) You can raise this if you'd like
# of shots to put peds in dying states:
2-3 shots to chest
3-5 shots to legs
3-6 shots to arms
1-2 shots to neck
1-2 shots to head (depends on if headbleed is installed)
INSTALLATION:

1. Download the PedDamageOverhaul.asi only from the link and drop it in your RDR2 main directory
2. Download my PedDamageOverhaul.ini attached here and drop in your main directory
Replace files when asked
IF USING HEADBLEED AND/OR BLOODLUST (Highly Recommended)
Bloodlust/Headbleed compatibility patch has already been applied
1. Delete peddamageinfo.meta files from:
\Red Dead Redemption 2\lml\HeadBleed_w_Euphoria
\Red Dead Redemption 2\lml\PDO v2.0 Extended Features\PDOHCFAB
2. Download my peddamageinfo.meta file attached here and drop it in
\Red Dead Redemption 2\lml\WhyEm's BloodLust\bloodHaemorrhage
Replace the file
